How to Choose the Best Lip Fillers Safely
Looking for the best lip fillers should enhance your lips without making them look obvious, heavy, or out of balance with your face. At Riverbanks Clinic, my approach is not simply to make lips bigger. Instead, I look at shape, projection, support, movement, facial proportions and ethnic background before I decide whether treatment is right.
Most patients who come to us from Harpenden, St Albans and Luton do not want an exaggerated lip filler look. They usually say the same thing in different ways.
“I want my lips to look fresher, but I do not want people to know.”
That is exactly where careful product choice and technique matter.
For many patients, my preferred lip filler is Restylane Kysse. I use it because it offers a balance of softness, flexibility, definition and natural movement. However, the product alone is not the result. The result comes from diagnosis, planning, placement and restraint.
The best lip filler is not the one that creates the biggest lip. It is the one that respects the face, supports the lip properly and still lets the mouth move naturally.

What makes a lip filler the best?
The best lip filler is not the same for every person. A good lip filler should:
- Add soft structure without stiffness
- Improve hydration without puffiness
- Respect natural movement
- Support the lip border when needed
- Avoid excessive forward projection
- Suit the patient’s age, face shape and heritage
- Reduce the risk of lumps when placed correctly
- Settle naturally over two weeks
Therefore, the question is not just, which filler is best? The better question is, which filler, in which layer, for which face, and for what purpose?
That is why Riverbanks Clinic uses a doctor-led assessment before lip enhancement. We assess the lips as part of the whole face, not as an isolated feature.
Why I recommend Restylane Kysse
Restylane Kysse is a hyaluronic acid dermal filler designed for lip enhancement and perioral lines. Hyaluronic acid is a water-binding molecule found naturally in the body. In dermal fillers, it helps create shape, support and hydration.
I recommend Restylane Kysse because it works well for patients who want a natural-looking result. It can improve lip softness, shape and definition while allowing normal expression.
Clinical studies have shown that Restylane Kysse can be effective and well tolerated for lip augmentation. In one randomised evaluator-blinded study, HA-RK, which refers to Restylane Kysse with lidocaine, showed long-term effectiveness and safety for lip enhancement. Another controlled multicentre study found it was effective through 48 weeks and well tolerated.
That does not mean everyone gets a 12-month result. Duration varies depending on metabolism, movement, dose, anatomy and previous treatments. However, in my experience, Kysse gives a pleasing balance between longevity and softness.
Why product choice alone is not enough
Patients often ask, do you use the best lip filler?
I understand the question. However, a filler is only one part of the treatment. The same product can look beautiful in one patient and poor in another if placed in the wrong plane, the wrong amount, or the wrong pattern.
Good lip filler treatment depends on:
- Facial assessment
- Lip anatomy
- Tooth show
- Smile pattern
- Chin projection
- Nose projection
- Age-related changes
- Previous filler
- Skin quality
- Patient expectations
So, although Restylane Kysse is my preferred product for many lip patients, the technique matters just as much.
My 3 layer lip technique
My 3 layer lip technique is designed to treat the lip as a three-dimensional structure. The lip is not a flat surface. It has different anatomical layers, and each layer plays a different role.

Broadly, I consider three treatment zones:
- Posterior support: this deeper layer helps support the lip from behind. It can improve structure without simply pushing the lip forward.
- Anterior shape and body: this layer helps refine lip body, softness and volume. It needs careful judgement because too much product here can look swollen.
- Supra-muscular refinement: this more superficial layer can help with contour, hydration and fine detail. However, it must be treated delicately.
This layered approach helps me avoid a one-dimensional filled look. Instead, I can use smaller amounts in more strategic planes.
Why I often use a cannula
A cannula is a blunt-tipped instrument used to place filler through a small entry point. I often use a cannula for lip enhancement because it can allow smoother product placement across tissue planes.
In suitable patients, cannula technique may help reduce multiple needle punctures. It can also be useful when I want to place product in a controlled, linear way.
However, it is important to be honest. A cannula is not automatically risk-free. Vascular complications can still occur with any filler technique. Therefore, safety depends on anatomical knowledge, careful technique, slow injection, low pressure, small volumes and constant assessment.
Current evidence suggests that technique, injector experience, product choice and anatomical awareness all matter. So, I do not use a cannula because it is fashionable. I use it when it gives the safest and most elegant route for that patient.
Why proportions matter for the best lip fillers
A good lip result should suit the rest of the face. Therefore, I assess the lips in relation to the nose, chin, jawline and facial profile.
One useful guide is the naso-mental line, which runs from the nose to the chin. It helps assess whether the lips are too recessed, too projected, or balanced in profile.
Another commonly discussed guide is Ricketts’ line. This is used in facial profile analysis to consider lip projection in relation to the nose and chin.
These lines are not rigid rules. They are guides. A face is not a mathematical formula. However, they help prevent over-projection, which is one of the most common reasons lip filler looks unnatural.
Why ethnic background changes the treatment plan
Not every patient should be treated to the same lip proportions. That is a critical point.
Different facial backgrounds have different natural lip shapes, volumes, projections and ratios. For example, some patients naturally have fuller lips, greater projection, or a different upper-to-lower lip relationship. Others have finer lips, more retrusion, or a different profile balance.
Research has shown measurable differences in lip morphology between ethnic groups. Therefore, a thoughtful lip enhancement plan should be tailored to the individual, rather than forcing one ideal lip shape onto every face.
This matters because poor lip filler often comes from copying a trend. A lip that looks balanced on one person may look completely wrong on another.
At Riverbanks Clinic, I do not aim to Westernise, exaggerate, or standardise lips. Instead, I aim to preserve identity while improving balance.
Why the upper lip should not always be bigger
Social media has created a preference for larger upper lips. However, this can be a mistake.
In many natural faces, the lower lip is slightly fuller than the upper lip. When the upper lip becomes too dominant, the mouth can look heavy, projected, or artificial.
That said, not everyone follows the same ratio. Some patients naturally have a fuller upper lip. Others need gentle upper lip support because ageing has caused flattening or loss of definition.
Therefore, I do not follow a fixed rule. Instead, I assess:
-
- Upper-to-lower lip ratio
-
- Smile animation
-
- Tooth show
-
- Cupid’s bow shape
-
- Philtral column support
-
- Vermilion border definition
-
- Chin and nose projection
-
- Overall facial harmony
This is especially important for patients who have had previous filler. Sometimes the best treatment is not more filler. Sometimes we need to dissolve old filler first.
The lip compartments matter too
The lips contain different anatomical compartments. These compartments influence how filler spreads, supports and settles.
If filler is placed without respecting these zones, the result can look uneven or bulky. It may also migrate above the lip border, creating the filler moustache effect that many patients fear.

The aim is not to fill every compartment. The aim is to understand which compartments need support, and which should be left alone.
This is where experience matters. Over-treating the lip is easy. Knowing when to stop is harder.
What results can patients hope to achieve?
With carefully planned lip enhancement, patients can often achieve:
-
- Better lip hydration
-
- Softer lip texture
-
- Improved border definition
-
- More balanced projection
-
- Gentle volume restoration
-
- Improved symmetry
-
- Better lipstick definition
-
- A fresher smile
-
- A more proportionate lower face
However, good results should still look like you.
Most patients do not want strangers to notice their filler. They want friends to say they look well, rested, or fresher.
That is the Riverbanks approach. We call it Invisible Aesthetics.
What happens during treatment?
Your appointment starts with assessment. We look at your face from the front, side and during movement. We discuss your goals, previous treatments and any concerns.
Then we decide whether filler is appropriate. If it is, we agree a plan. This may involve treating the lips only. However, in some patients, chin support, skin quality or perioral lines may also need consideration.
During treatment, we cleanse the area carefully. We then use local measures to improve comfort. Restylane Kysse contains lidocaine, which helps reduce discomfort during treatment.
I usually inject slowly and conservatively. In many cases, I prefer a cannula-led approach. However, I may use a needle for small areas of precision when needed.
After treatment, we review the lips and give aftercare advice.
What is the recovery like?
Most patients have some swelling. This is normal. The lips are vascular, mobile and sensitive, so they often swell more than other areas.
Typical recovery looks like this:
-
- Day 1: swelling, tenderness and possible unevenness
-
- Day 2 to 3: swelling may peak
-
- Day 4 to 7: swelling begins to settle
-
- Day 7 to 14: shape becomes more natural
-
- After 2 weeks: final result becomes easier to judge
Bruising can happen. Small lumps can also occur early, but they often soften as swelling settles. However, severe pain, blanching, dusky colour change, increasing swelling, or skin changes need urgent review.
Why less is often better for the best lip fillers
The best lip fillers rarely look impressive immediately. In fact, they often look slightly underwhelming at first.
That is a good thing.
Lips can swell after treatment, and patients sometimes like the swollen phase. However, chasing that swollen look with more filler can quickly lead to heaviness, migration and loss of natural movement.
Therefore, I prefer staged treatment when needed. We can always add more later. It is much harder to correct lips that have been overfilled repeatedly.

The Best Lip Fillers Summary
The best lip fillers are not about maximum volume. They are about proportion, softness, movement and identity. For many patients, I recommend Restylane Kysse because it gives a natural balance of structure, flexibility and refinement. However, the real difference comes from the treatment plan, the 3 layer technique, careful cannula use and a proper understanding of facial proportions.
