Skin Changes After GLP-1 Weight Loss

1. Introduction

Skin changes after GLP-1 weight loss can include facial volume loss, loose skin, reduced firmness and changes to the shape of the face or body.

Medicines such as Mounjaro, Wegovy and other GLP-1-based treatments can support significant weight loss. For many patients, this brings important improvements in health, confidence and quality of life.

However, the skin and underlying tissues may not adapt at the same speed as the weight loss. As a result, some people notice that their face looks more tired, hollow or less supported. Others develop loose or crepey skin on the face, neck or body.

At Riverbanks Clinic, we provide personalised, doctor-led treatment plans to support the skin, restore appropriate facial balance and encourage collagen production.

Our aim is not to reverse the benefits of weight loss or make the face look artificially full. Instead, we use our Invisible Aesthetics approach to help you look healthier, fresher and still recognisably yourself.

2. Overview

Medication-driven weight loss can affect the skin, superficial fat, deeper facial fat and the overall balance of the face.

These changes are not the same as lipoatrophy. Lipoatrophy is a medical condition involving abnormal or localised loss of fatty tissue. By contrast, the changes seen after GLP-1 weight loss usually form part of wider body and facial fat reduction.

The appearance will vary according to several factors, including:

  • Age
  • Gender
  • Ethnicity
  • Skin quality
  • Facial structure
  • The amount of weight lost
  • The speed of weight loss
  • The amount of weight still expected to be lost

The superficial facial fat compartments may show the earliest visible changes. However, both superficial and deeper fat compartments may be affected.

Current research has not yet established exactly which compartments are affected most in every patient. Therefore, treatment should always follow a detailed facial and medical assessment.

The face and body require different approaches

The face contains small fat compartments, muscles, ligaments and delicate anatomical structures. Even small changes can affect facial expression, proportions and identity.

Body concerns often involve larger areas of loose skin, such as the abdomen, arms, thighs or buttocks. Therefore, the treatments and expected results may be different.

For severe excess body skin, surgery may provide a more meaningful result than non-surgical treatment.

When should treatment begin?

It is not always necessary to wait until you reach your final target weight.

In suitable patients, collagen-stimulating treatment such as Sculptra may begin while weight loss is still taking place. This may help support the skin during the period of change.

However, more substantial structural correction may be better delayed until the rate of weight loss has slowed and the likely final facial shape is clearer.

3. Reviews

Patients are often very pleased with their improved health and weight. However, they may feel disappointed that their face appears more tired or older than they feel.

Common concerns include:

It may be particularly helpful when weight loss has caused widespread facial deflation or reduced skin firmness.

In suitable patients, Sculptra can be started while weight loss continues. Treatment is usually staged, with progress reviewed over several months.

Hyaluronic acid dermal fillers

Dermal fillers may be used to restore selected areas of structural support.

We do not aim to replace every millilitre of lost facial fat. Excessive filler can make the face appear heavy or unnatural.

Secret RF uses radiofrequency microneedling to deliver controlled energy beneath the skin.

Cool Laser resurfacing

Cool Laser uses Er laser technology to resurface the skin precisely.

Cool Laser treats the skin rather than replacing deeper facial volume. Therefore, it may be combined with collagen stimulation or carefully selected injectable treatment.

Skin boosters and medical skincare

Skin boosters can improve hydration, elasticity and luminosity.

Medical skincare may also help protect collagen, support the skin barrier and improve overall skin quality.

However, creams and skin boosters cannot replace significant lost volume or remove substantial excess skin.

We will not advise you to stop or alter prescribed medication. Any changes to your weight-management treatment should be discussed with your prescribing clinician.

Facial and skin assessment

Sculptra: Results develop gradually over several weeks and continue to improve as new collagen forms.

Cool Laser: Initial improvement appears after the skin has healed, followed by continued collagen remodelling.

Combination treatment: Results develop in stages because each treatment works on a different layer of the face or skin.

Both may be affected.

Superficial fat loss may become visible first. However, current research has not yet confirmed which facial fat compartments are affected most in every patient.

Why are face and body treatments different?

The face contains small fat compartments, muscles and delicate anatomical structures that influence expression and identity.

The body usually involves larger treatment areas and may have more significant loose or hanging skin. Therefore, the treatments and expected results are different.

Can fat grafting be used?

Fat grafting may be considered by a plastic surgeon once weight is stable.

However, further weight loss may reduce the transferred fat. Weight regain may also enlarge it and alter the result.

For this reason, fat grafting is normally considered only when the patient’s weight and medication plan are stable.

Will dermal filler make my face look puffy?

Excessive or poorly planned filler can make the face appear heavy.

At Riverbanks Clinic, we use filler selectively to restore structural support. We may recommend collagen stimulation or skin treatment instead of adding larger amounts of filler.

What downtime should I expect?

Downtime depends on the treatment.

Injectables may cause temporary swelling, bruising or tenderness. Secret RF may cause redness, swelling and a rough skin texture for several days. Cool Laser can cause redness, swelling, peeling or bronzing, depending on the treatment strength.

You will receive full aftercare instructions.

What are the risks and is treatment safe?

All medical aesthetic treatments carry risks.

Injectable risks include swelling, bruising, infection, asymmetry, lumps, nodules and an unsatisfactory result. Dermal filler can rarely block a blood vessel, potentially causing skin injury or, very rarely, visual complications.

Energy-based treatments can cause burns, prolonged redness, infection, pigmentation changes or scarring.

Careful assessment, appropriate treatment selection, sterile technique and access to medical support help reduce these risks.

Does medication-driven weight loss affect facial bones?

There is no current evidence that the visible facial changes are caused by loss of facial bone.

The most relevant aesthetic changes occur within the skin, fat and soft tissues. Wider bone health should be assessed medically in patients who have osteoporosis risks or substantial weight loss.

Can loose body skin be treated without surgery?

Mild or moderate loose skin may improve with radiofrequency microneedling or other energy-based treatments.

However, significant hanging or folded skin may require surgical removal. We will be honest when a non-surgical treatment is unlikely to produce a worthwhile result.
