What Happens During a Profhilo Structura Treatment?

What to Expect from Profhilo Structura: Treatment, Results, and Side Effects Explained

Quick answer

Profhilo Structura treatment is a regenerative injectable containing 90mg of hyaluronic acid in 2ml. Unlike standard Profhilo, it is injected into the superficial facial fat layer to support age-related volume loss. Most patients require two treatments, usually four weeks apart. Results appear gradually over 2 to 3 months and typically last around 9 to 12 months.

If you’ve been considering Profhilo Structura treatment to address sagging, facial hollowness or early structural ageing, understanding exactly what’s involved can help you feel more confident in taking the next step.

Profhilo Structura Summary

  • Profhilo Structura is an advanced hyaluronic acid injectable treatment.
  • It is designed to support areas affected by age-related facial fat loss.
  • It targets the superficial fat layer, rather than only the skin surface.
  • It contains 90mg of hyaluronic acid in 2ml.
  • By comparison, standard Profhilo contains 64mg of hyaluronic acid in 2ml.
  • Therefore, Structura contains about 41% more hyaluronic acid than standard Profhilo.
  • Results are usually subtle, gradual and natural-looking.
  • Most patients need two treatments, usually around four weeks apart.
  • Side effects are usually mild, although all injectable treatments carry risks.
  • At Riverbanks Clinic, treatment is doctor-led and medically supervised.

What Is Profhilo Structura?

Profhilo Structura is an advanced injectable treatment designed to support facial structure in areas affected by age-related fat loss. It is injected into the subcutaneous fat layer below the skin, where it may help improve facial support, firmness and contour.

Many patients notice this type of ageing as cheek hollowing, flattening at the sides of the face, or early sagging. However, the problem is not always simply loose skin. Often, there is also a change in the quality and position of the deeper facial fat compartments.

Standard Profhilo is best known for improving skin quality, hydration and elasticity. By contrast, Profhilo Structura is designed to work at a deeper structural level. It uses stabilised hybrid cooperative complexes of high and low molecular weight hyaluronic acid, produced using IBSA’s NAHYCO technology.

Although it can improve facial support, Profhilo Structura is not the same as a traditional dermal filler. It is not usually chosen to create sharp cheekbones, dramatic projection or a visibly sculpted appearance. Instead, it is used when the goal is softer, more natural structural support.

At Riverbanks Clinic, we use Profhilo Structura as part of our Invisible Aesthetics approach. In other words, the aim is not to change the face. Instead, the aim is to help patients look fresher, less hollow and more like themselves again.

Dr Ravi Jain explains: “I tend to use Structura when the issue is not just the skin surface, but the support underneath. The best result should not look like filler. It should simply make the face look less tired and better supported.”

In this video, Dr Ravi Jain explains the difference between Profhilo and Profhilo Structura, and why the correct treatment depends on whether the concern is skin quality, facial support or both.

What Happens During a Profhilo Structura Treatment?

1. Personalised Consultation

Firstly, we assess your face carefully. We look at facial shape, volume loss, skin quality, laxity, symmetry and your natural ageing pattern.

We also review your medical history. This includes allergies, medication, previous injectable treatments, autoimmune conditions, pregnancy status and any history of complications.

This matters because Profhilo Structura is not right for everyone. For example, some patients need collagen stimulators, dermal fillers, laser resurfacing, radiofrequency microneedling or a combined treatment plan instead.

At Riverbanks Clinic, the consultation is not about persuading every patient to have treatment. Instead, it is about deciding whether the treatment is genuinely suitable and safe.

2. Clinical Assessment and Treatment Planning

Next, we identify whether you are more of a “sinker” or a “sagger”. These are the two facial ageing patterns commonly discussed with Profhilo Structura.

  • Sinkers tend to have hollowing or flattening, particularly around the lateral cheek.
  • Saggers tend to show tissue descent, heaviness or early drooping.

However, most faces do not fit perfectly into one category. Therefore, we plan treatment according to your anatomy, rather than using a one-size-fits-all approach.

3. Skin Preparation

The skin is cleansed thoroughly before treatment. This reduces infection risk and creates a safer treatment environment.

Topical anaesthetic may be used, although many patients tolerate the procedure well without it. You may feel pressure, mild stinging or a pulling sensation, particularly if a cannula technique is used.

4. Injection Technique

Profhilo Structura may be injected using either a cannula or a needle. The choice depends on your anatomy, the treatment area and the safest route of placement.

  • Cannula technique may be preferred for broader areas and may reduce bruising risk.
  • Needle technique may be used when more precise placement is required.

The product is placed into the superficial fat layer. As a result, it works differently from standard skin boosters, which usually focus more on skin hydration and surface quality.

5. Profhilo Structura Aftercare Guidance

After treatment, we give clear aftercare advice. This helps reduce unnecessary swelling, irritation and bruising.

  • Avoid makeup for at least 12 hours.
  • Avoid touching or massaging the treated area.
  • Avoid alcohol for 24 hours.
  • Avoid strenuous exercise for 24 hours.
  • Avoid saunas, steam rooms and sunbeds for 48 hours.
  • Avoid facial treatments for at least one week.
  • Contact the clinic if anything feels unusual or concerning.

Our Experience with Profhilo Structura at Riverbanks Clinic

Since introducing Profhilo Structura at Riverbanks Clinic, we have used it mainly in patients with early facial hollowing, menopause-related volume changes and, more recently, facial changes following GLP-1 weight loss.

We have now performed approximately 800 Profhilo Structura treatments. Around 45% of patients have Structura as a standalone treatment, while around 55% combine it with another treatment such as Sculptra, Secret RF, Cool Laser, VBeam or standard Profhilo.

Although every face ages differently, most patients require two treatments. Many remain happy for 9 to 12 months before considering maintenance.

Dr Ravi Jain explains: “One thing I’ve noticed is that patients rarely come in asking for bigger cheeks. They usually say they look tired, hollow or less like themselves. Structura works well when the goal is subtle support rather than obvious volume.”

What I’ve Learned from Using Profhilo Structura

One of the most interesting groups of patients has been those who have lost weight with medications such as Mounjaro, Wegovy or Ozempic. Their concern is often not wrinkles. Instead, they feel their face has become flatter, more hollow or less supported.

Another observation is that patients who dislike the look of traditional fillers are often drawn to Structura. They want improvement, but they do not want to look different.

In younger patients, especially those in their forties, two treatments may be enough. However, patients in their fifties and sixties may need a more layered plan, particularly if they have skin laxity, collagen loss and volume loss at the same time.

The best compliment I hear is, “I look better, but nobody can tell what I’ve had done.” In my opinion, that is exactly what modern aesthetic medicine should aim to achieve.

How Many Sessions Are Required?

Most patients need two treatment sessions. These are usually spaced around four weeks apart.

Some patients may benefit from a third treatment. This depends on the degree of hollowing, tissue quality, age, previous weight loss and the result after the initial course.

A typical Profhilo Structura plan may include:

  • Initial medical consultation.
  • First treatment session.
  • Second treatment session four weeks later.
  • Review after results have developed.
  • Maintenance every nine to twelve months, where appropriate.

However, maintenance should not be automatic. At Riverbanks Clinic, we review your result first. Then we decide whether another session is useful.

How Long Do Results Really Last?

Clinical studies suggest that improvement can continue for several months following treatment. However, in everyday practice, I advise patients to think of Profhilo Structura as an annual treatment rather than a permanent solution.

Most patients enjoy results for around 9 to 12 months. Nevertheless, ageing continues and factors such as weight loss, menopause, sun exposure, smoking, stress and illness can all influence how long the benefits last.

Some patients choose yearly maintenance. Meanwhile, others combine Structura with treatments such as Sculptra, Secret RF, standard Profhilo or laser resurfacing to maintain facial support and skin quality over the longer term.

What Results Can You Expect?

Profhilo Structura provides subtle and natural-looking results. Most patients do not look “done”. Instead, they often look a little fresher, less hollow or better supported.

Results usually build gradually over 6 to 12 weeks. Therefore, this is not the right treatment if you want an instant dramatic change.

Depending on your anatomy and treatment plan, you may notice:

  • Improved midface support.
  • Softer cheek hollowing.
  • Smoother facial contours.
  • Better facial firmness.
  • A less tired appearance.
  • Improved skin texture and elasticity.
  • Subtle support around early lower-face heaviness.

However, results vary between patients. Age, skin quality, menopause, sun damage, weight loss, stress, illness and previous treatments can all affect the final outcome.

Profhilo Structura for Ozempic Face and Mounjaro Face

Rapid weight loss can have many health benefits. However, some patients notice that their face appears more hollow, tired or less supported after losing weight with medications such as Mounjaro, Wegovy or Ozempic.

Although the media often calls this “Ozempic Face”, the issue is not caused by one specific medication. Instead, it reflects the loss of facial fat that can happen after significant weight reduction.

Profhilo Structura has become one of my preferred treatments for selected patients in this group because it provides subtle support without creating an overfilled appearance. In many cases, the aim is not to replace every bit of lost volume. Instead, the aim is to restore a fresher and healthier appearance.

Patients who have lost larger amounts of weight may benefit from combining Structura with collagen stimulators such as Sculptra. Meanwhile, treatments such as Secret RF and Cool Laser can improve skin firmness and texture.

The overall goal is to help patients enjoy the health benefits of weight loss without feeling that they have aged prematurely.

What Are the Side Effects?

Most side effects with Profhilo Structura are mild and short-lived. Nevertheless, it is still an injectable medical treatment. Therefore, risks must be discussed properly before treatment.

Common side effects include:

  • Redness at the injection site.
  • Temporary swelling.
  • Mild bruising.
  • Tenderness.
  • Small temporary lumps.
  • A feeling of pressure or tightness.

These usually settle within a few days. Occasionally, bruising may last longer.

Less common side effects include infection, inflammation, delayed swelling, persistent lumps or an allergic-type reaction.

Rarely, injectable treatments can affect a blood vessel. This can cause skin injury and, very rarely, visual symptoms. Although this is uncommon, it is important to recognise early warning signs.

Please contact the clinic urgently if you notice:

  • Severe or worsening pain.
  • Pale, grey, mottled or dusky skin.
  • Blistering.
  • Marked asymmetry with increasing swelling.
  • Visual disturbance.
  • Symptoms that feel unusual or severe.

At Riverbanks Clinic, we follow rigorous safety protocols to minimise risk. We also ensure that patients receive clear aftercare advice and know how to contact us after treatment.

In simple terms, Profhilo improves the skin canvas. Profhilo Structura supports the deeper facial architecture. Sometimes we use one. Sometimes we combine both.

Is Profhilo Structura the Same as Filler?

No. Profhilo Structura is not the same as a traditional dermal filler.

Dermal fillers are often used to create contour, replace lost volume or define areas such as cheeks, lips, chin or jawline. They can be excellent when used carefully. However, they can also look obvious when overused.

Profhilo Structura is different. It is a hyaluronic acid injectable intended to support tissue quality and facial structure. It does not usually create the same projection or sculpting effect as filler.

Therefore, the right choice depends on your face and your goals. If you need clear shape restoration, a filler or collagen stimulator may be more appropriate. If you need subtle support without obvious volume, Structura may be suitable.

Who Is Profhilo Structura Best For?

Profhilo Structura may suit patients who want a natural-looking improvement without looking filled or altered.

It may be suitable if you have:

  • Mild cheek hollowing.
  • Early facial flattening.
  • Subtle sagging.
  • A tired or drawn appearance.
  • Loss of soft tissue support.
  • Skin that feels less firm.
  • A preference for gradual results.
  • Facial changes after weight loss.
  • Menopause-related facial volume changes.

Who Is Not Suitable?

Profhilo Structura is not always the best option. It may not be enough if you have significant skin laxity, marked jowling, deep folds or major volume loss.

It is also not suitable if you are pregnant, breastfeeding, have an active skin infection in the treatment area, or have certain medical conditions that make injectable treatment unsafe.

In some cases, we may discuss other Riverbanks treatments, such as Profhilo, dermal fillers, Sculptra, Ellansé, Secret RF, Cool Laser or a layered plan.

Common Mistakes I See with Profhilo Structura

Expecting filler-like results

Profhilo Structura is subtle. Patients wanting dramatic cheek projection may be disappointed. Therefore, we always discuss expectations carefully before treatment.

Treating severe sagging with Structura alone

In advanced ageing, Structura may not be enough on its own. In these cases, a layered approach may work better.

Chasing volume instead of support

Sometimes patients think they need more volume. However, support, skin quality and collagen stimulation may be more important.

Not allowing enough time

Results are gradual. Most patients notice improvement over 8 to 12 weeks, rather than immediately after treatment.

How I Use Profhilo Structura in Practice

I rarely think about Profhilo Structura as a simple one-product solution. Most faces do not age in one dimension. Patients lose support, collagen, skin quality and elasticity at the same time.

Therefore, I often combine Structura with other treatments, depending on what is driving the ageing process.

Concern Possible Treatment Plan
Early hollowing Profhilo Structura
Hollowing with skin laxity Profhilo Structura with Secret RF
Weight-loss face Profhilo Structura with Sculptra
Skin quality concerns Profhilo Structura with standard Profhilo
Advanced ageing A layered regenerative treatment plan

Dr Ravi Jain explains: “If you want people to say you’ve had something done, Structura probably is not the right treatment. However, if you want friends to say you look well, less tired or ask whether you’ve been away, then it fits perfectly with our Invisible Aesthetics philosophy.”

Frequently Asked Questions

How painful is Profhilo Structura?

Most patients describe the treatment as mildly uncomfortable rather than painful. Treatment is quick and no significant downtime is usually required.

Can Profhilo Structura replace dermal fillers?

Not completely. Structura provides subtle support and regeneration. However, patients seeking more dramatic volume restoration may still benefit from fillers or collagen stimulators.

Does Profhilo Structura dissolve fat?

No. Profhilo Structura does not dissolve fat. Instead, it is designed to support areas affected by age-related changes in the superficial fat layer.

How many treatments will I need?

Most patients require two treatments approximately four weeks apart. Maintenance is usually performed every 9 to 12 months, depending on response.

Can Profhilo Structura be combined with Sculptra?

Yes. In my practice, combining Structura with Sculptra can work particularly well in patients with significant age-related volume loss or facial changes following weight loss.

Is Profhilo Structura suitable after Ozempic, Wegovy or Mounjaro?

Yes, it may be suitable. Patients who have experienced facial hollowing after weight loss are often good candidates, although some may need a more comprehensive treatment plan.

At what age should I start?

There is no perfect age. Most patients begin treatment in their forties or fifties when early structural changes become noticeable.

Can men have Profhilo Structura?

Yes. Male patients often appreciate the subtle and natural results because treatment does not alter masculine facial characteristics.

How long does swelling last?

Minor swelling usually settles within 1 to 3 days, although occasional bruising can last slightly longer.

What happens if I stop treatment?

Your face will simply continue to age naturally. Profhilo Structura does not make the skin worse if treatment is discontinued.

Scientific Evidence

The evidence for Profhilo Structura is promising, although still developing. Therefore, it is important to describe the treatment responsibly.

  • Cassuto et al. (2023) reviewed initial clinical experience with Profhilo Structura and reported integration within facial fat compartments, with clinical improvement observed during follow-up.
  • Sparavigna et al. (2024) published an observational pilot study assessing Profhilo Structura for restoration of the lateral cheek fat compartment, reporting favourable efficacy and tolerability findings.
  • Vassallo et al. (2024) examined highly concentrated hyaluronic acid hybrid complexes and their compatibility with adipose tissue and adipose-derived stromal cells in laboratory conditions.
  • General Medical Council guidance explains the standards expected from doctors who offer cosmetic interventions, including consent, training, safety and realistic expectations.
